The Long View with: Dr. Vonda Wright, MD

This week, we're handing the newsletter over to Dr. Vonda Wright, MD, orthopaedic sports surgeon, CEO of Precision Longevity, and NYT bestselling author of Unbreakable, as part of our series The Long View, where leading voices in women's health share what they believe matters most for how we age.

If you listen to the loudest voices in the longevity space today, you will hear a familiar litany of metrics. We are told to optimize our VO2 max, track our ApoB levels, monitor our continuous glucose monitors, and biohack our sleep architecture. We are chasing cardiovascular endurance, metabolic flexibility, and cognitive preservation.

We are told, quite rightly, that exercise is the master key to all of this. Exercise is the pill that prevents cognitive decline, strengthens the heart, and regulates metabolism. But as an orthopaedic surgeon who has spent three decades operating on the consequences of aging, I can tell you with absolute certainty that the longevity community is missing the foundation.

Literally. They are missing the foundation.

  • Skeletal Longevity is the gateway to every other kind of longevity. If your bones, joints, and tendons can't support movement, you can't get the exercise that protects your heart and brain.

  • Your musculoskeletal system is a full ecosystem (bone, cartilage, tendon, ligament, disk, muscle), and it's built by peak bone mass in your early thirties, then challenged by accelerated loss during perimenopause and menopause.

  • Frailty isn't inevitable. Mechanical loading (heavy lifting, impact exercise), training with female biomechanics in mind, and getting enough protein, vitamin D, K2, calcium, and hormonal support can keep the "gate" open.

You can have the cardiovascular engine of a 30-year-old and the cognitive sharpness of a chess grandmaster, but if your skeleton cannot support your body weight—if your knees will not bend, if your spine cannot twist, if your tendons tear under load—your world shrinks to the size of your living room.

I call this concept Skeletal Longevity, and it is the prerequisite for every other kind of longevity you care about. It is the critical gate through which all performance, independence, and vitality must pass. If the gate is closed, the journey stops.

The Ecosystem of the Skeleton

When I say “Skeletal Longevity,” I am not just talking about your bones. Your skeleton is an entire, interconnected musculoskeletal ecosystem. It is bone, yes, but it is also muscle, cartilage, tendon, ligament, spinal disk, and fat.

When any single component of this ecosystem fails, the entire structure is compromised.

The Bone Foundation: This is where it starts. Your bones are not static scaffolding; they are living, dynamic tissue constantly remodeling itself. From childhood through your late twenties, you are making massive deposits into your bone density bank account. By your early thirties, you hit your peak bone mass. For the rest of your life, you are living off that peak. If you reach your peak with low bone density, the inevitable withdrawals of perimenopause and menopause—when estrogen drops and bone loss accelerates—can push you straight into osteoporosis. When a fragile bone breaks, it is not just an injury; it is a catastrophic loss of independence.

The Cartilage Crisis: Consider osteoarthritis. Women suffer from osteoarthritis at significantly higher rates than men. Cartilage is the smooth, frictionless surface that allows your joints to glide. When it wears away, bone grinds on bone. The pain is excruciating. The stiffness is paralyzing. If your hips and knees are locked in arthritic pain, you cannot run. You cannot lift. You cannot perform the very cardiovascular exercise required to keep your heart and brain young. The gate is closed.

The Tendon and Ligament Tethers: Tendons connect muscle to bone; ligaments connect bone to bone. As estrogen declines during midlife, these connective tissues lose their stiffness and resilience. They become vulnerable. You might have the muscular strength to lift a heavy weight, but if your tendon cannot tolerate the load, it tears. A ruptured Achilles or a torn rotator cuff can sideline you for a year, accelerating the decline of every other system in your body.

The Spinal Disks and Muscular Engine: Your spinal disks provide the shock absorption that allows you to stand upright and move with agility. Your muscles are the engine that drives the entire system, but as we age, we lose muscle mass (sarcopenia) and the muscle we keep becomes infiltrated with fat, changing its quality and function.

The Illusion of the Inevitable Decline

For generations, the medical establishment has treated the musculoskeletal decline of women as an inevitable consequence of aging—a slow, unavoidable crumbling that we must simply accept. We have normalized frailty. We have been told that shrinking, breaking, stiffening, and slowing down are just the tax we pay for getting older.

I reject that premise entirely.

Your musculoskeletal ecosystem is highly responsive. It requires specific signals to maintain its integrity. If you do not provide those signals, your body assumes you no longer need a strong skeleton and stops investing energy in it.

The Unbreakable Mandate

Whether you are 35 and still building your peak, or 55 and fiercely protecting what you have, the mandate of Skeletal Longevity remains the same. You must give your ecosystem a reason to stay strong.

1. Mechanical Loading is Non-Negotiable

Bones, tendons, and muscles respond to the physical demands placed upon them. You must lift heavy weights. You must engage in impact exercises if your joints allow it. Walking is wonderful for your heart and your mind, but it is not enough to build bone or fortify tendons. You must load the system.

2. Respect Your Biomechanics

Female biology is not a footnote—it is the headline. We have wider hips (a larger Q angle) and different upper body geometry (a larger carry angle). We must train in alignment with our anatomy to protect our cartilage and ligaments from nnecessary wear and tear.

3. The Nutritional and Hormonal Matrix

You need adequate protein to form the collagen matrix of your bones and maintain your muscle mass. You need Vitamin D, Vitamin K2, and calcium. And you must understand your hormonal baseline, discussing the role of Menopausal Hormone Therapy (MHT) or other interventions if your ecosystem is at risk.

Redefining the Trajectory

Skeletal Longevity is not about fear; it is about empowerment. It is about recognizing that you have agency over the structural integrity of your body.

You are not destined to become frail. You are not destined to be sidelined by stiff joints or fragile bones. By understanding the science of your musculoskeletal ecosystem and taking decisive, proactive action, you can keep the gate open. You can build a body that carries you through every decade with strength, power, and grace.

This is the foundation. This is where we start. Age with Power.
